After your individual appointment, you will receive an invoice for payment. Class packages are paid for at the time of purchase.

4:8 Physio is CASH-BASED and does not have any relationship with any insurance institution or company whatsoever.

“Cash-Based” means that 4:8 Physio does not accept any forms of insurance. The patient is responsible for 100% of the costs incurred from any treatments and services.
However, 4:8 Physio can provide Superbills to patients upon request. These Superbills may be provided to the patient who may then present that Superbill to their own insurance provider for potential direct reimbursement.

A Superbill is a common billing document that includes an itemized list of all services provided to a patient with applicable care codes and other treatment information. A patient can provide their Superbill to their insurance company to show what they have paid to 4:8 Physio for treatment, and the patient’s insurance may reimburse them for their costs.

Any reimbursement of any kind is NOT guaranteed and depends solely on your insurance company and applicable benefits. 4:8 Physio is not able to reach out to your insurance company to discuss your benefits. Any reimbursement submissions are conducted outside of 4:8 Physio’s involvement, and in the patient’s sole discretion.

We are pleased to assist you with the superbill process. Your first superbill is complimentary, with no fees applied. Subsequent superbills will incur an hourly administrative fee of $25. This fee covers our time spent gathering necessary information, including diagnostic codes, ICD-10 and CPT codes, as well as researching your insurance details, communicating with insurance companies, corresponding with other healthcare providers, and all other activities required to facilitate the superbill process.
The same administrative rate applies to any correspondence with legal offices, records requests from other healthcare providers, compiling notes and invoices, and other administrative reports. This fee schedule does not apply to your daily written session notes or any of our regular operational tasks. You will receive an administrative invoice for these services once completed; these services are not eligible for insurance reimbursement, and payment is due upon receipt of the invoice.

You may request a Superbill from 4:8 Physio as often as you would like. 4:8 Physio encourages patients to work directly with their insurance companies to determine how often they will need to request a Superbill for potential reimbursement.

Superbills are only provided upon patient request. After requested, Superbills will be provided to the patient via email within 2-3 weeks of the patient’s request.

Due to the resources required to generate these documents, a $25.00 preparation fee will be charged for each generated Superbill.
